Anyway to answer your questions.
1) He needed a blindfold to stay in his true form. The darkness changed his outward appearance when he had it off because, and this is just a theory, he welcomed it into his heart. He hadn't always let it take him over completely, but he wasn't afraid of it anymore. The blindfold represents darkness and his body, I think, represents light. When he has it on, his body is his, from the realm of light. When he takes it off, and he can see light, his body takes the shape of the darkness in his heart, Ansem SoD.

2) It hasn't been explained but maybe he's just ninja liek that! XP

3) Nomura wished it, so Nomura made it happen...

 

To answer your second and third question, i believe it may be like various other movies and games, where the character uses some sort of dark power to "feel" exactly the area around them. We already know he can smell darkness (KH:CoM), but there may be other senses influenced by it also.

Maybe Diz is secretly feeding him information as he fights. "Riku, he's attacking from your left" and things like that?

 

1: in the end of re:com when they give him the cloak (i forget who) they say it will protect him from the darkness. i always figured the blindfold was like an extension of the cloak and further protected him.

Alright, I know you've gotta couple of answers already, but I'll just join the bandwagon...

1) Riku's Blindfold - The reason he wears it is to keep track of his true physical form. Without it, he'd just be running around as Ansem SoD all the time. the blindfold basically suppresses his inner emo.
2) Being Able To See - There's a slight chance that maybe the blindfold is semi-transparent, but I doubt it. I wanna say the actual reason Riku can see is because of his sense of smell, which he was revealed to have in Chain of Memories. I'm fairly sure it's strong.
3) Kicking the butt of Roxas - He had Ansem SoD watching his back, bros forever man. (in a sense) Roxas got double teamed. (once again, in a sense)

 

I always thought he could see through the blindfold without any problem. Although, he keeps his eyes close under, so it might be his sense of smell, but I don't know.

 

For the second, it's his smell.
sorta like Toph from Avatar can see with her feet, Riku can see by smelling the darkness.
Which is why he was confused when he fought Xion for the first time, and had to see her face with his eyes to recognize who it was.

 

I wonder who he saw when he looked at her face.

2) It's probably like in other games when they use every other sense, smell, sound, touch, 'feeling', etc. Or Nomura could've just had a thing where Riku could've seen through it, because honestly, you never know with Kingdom Hearts.
3) If Nomura says so, it goes in the game.

Actually, I'd like to know that too. If I had to say at first, he probably saw Sora, but later in the game, when the two met again, he probably viewed her as Xion.

Like the others said I think because of Riku's dark powers he's able to sense and feel around him without seeing(like echolocation that doesn't involve screaming at everyone...that would have been amusing..) He might just be able to see through it though.

The blind fold seems to be Riku's little secret. The biggest explination we get is this.

So Riku used the blind fold to lie to himself in some way. Perhaps in some odd way not being able to see allowed him to believe that his darkness wasn't as bad as it was.

As for how he can maneuver and fight, we know from CoM that he can "smell" people. I don't think smell was supposed to be taken literally but just being able to sense people's energy. I think it is safe to say that even by the time he fights Xion he had fine tuned that skill.

 

What you basically said was that the blindfold was used because he didn't want to face that truth that was in his eyes, so he used something that would keep his eyes hidden from him
