    I don't think we'll ever see a major breakthrough until the graphics improve significantly. Games are still too uncanny valley right now. It's probably the worst it's ever been, with the exception of L.A. Noire.

    Like I said, its not the graphics that are the make-or-break to me, its the artistic designs. PC graphics right now can be freaking amazing. Now all we need is someone to figure out how to utilize them to make really creepy creatures.

    That actually gave me several ideas. Delay based combat could allow for incredibly scary late game encounters when the player realizes that (s)he's been fighting the same enemies for the past five to ten gameplay hours, each time stronger and more determined to kill the hell out of them. Even worse would be that they can come back randomly at small but well spaced intervals (say, max of ten minutes with lots of nothing in between). The only safe points would be story based events, and those would be few and far between.

    Rare health items would be okay, but even better would be making them rare and inconvenient to pick up or use. In particular, they'd be unusable if you are holding your weapon, which must be dropped while you recover health and then picked back up. Combined with random encounters that can happen frequently, you're screwed if you take more than twenty to thirty seconds to heal.

    The best way to make enemies ambiguous would be to ensure that they can't actually be seen, at least not for long. Have them show up, make themselves known, and then disappear from sight so the player has to fight them without any visual cues about where they are.

    And finally, a huge open world to really hammer in how alone you are and the fact that anything can get you anywhere at any time. There will be opportunities to sleep, but if an enemy attacks you, you're dead in an instant.
