    It is in no way fake as it's in an official trailer. Sure Hideo has a weird sense of humour, but that isn't it.

    You really think Kojima would EVER reveal a plot twist in a trailer? It's Kojima. He loves tricking people. It's probably something simple like Eva being reminded of Big Boss in Snake and imagining his face, which would explain the cigar, since again, Snake doesn't smoke cigars.
