    Organ Questionnaire

    1. How old are you? 16

    2. Where do you live? (Country)USA

    3. On a scale of 1 to 10 how much do you know about organ/tissue donation? Maybe a 6

    4. What information would you like to know or would you have liked to have known more of regarding organ donation? Isn't there a way that you can be an organ donor but only donate them once you're deceased?

    5. Are you an organ donor? Why or why not? Not currently. Parents decided they don't want me to be one.

    6. Did your knowledge or lack of knowledge effect your decision? If so which one and why? It didn't affect my decision. I'm pretty sure that I'll be one when I'm of age.

    7. Do you know anyone who has donated/received an organ/tissue? Not that I'm aware of

    8. Are your parents organ donors? If not, do you know why? I don't think they are and I've no idea as to why they aren't

    9. Did your parents influence your decision to be or not an organ donor? No

    10. Do your parents know your wishes on organ donation? Not currently

    11. Did/Are you going through Drivers Ed through your local DMV office? Or through an online course or home study? I'm working on it.

    12. During the course did they mention organ donation?Yes.

    13. Do you have a Drivers Permit or license?Temps

    14. How long when you got your permit were you given to make a decision if you wanted to be an organ donor? Uh not very long since parents decided for me.

    15. Did you know before you went to the DMV that they would ask you about becoming an organ donor? Nope

    16. Where do you think it would be beneficial to put information about
    organ donation for new drivers? The local DMV office, Health classes, etc.
    Any other suggestions? Probably the DMV office and a Health class

    3. On a scale of 1 to 10 how much do you know about organ/tissue donation? Probably around a 9 or 10.

    4. What information would you like to know or would you have liked to have known more of regarding organ donation? I learned more in college studying science, so yeah, it doesn't hurt that students in high school could learn of how useful tissue, skin and organ donation is and who it benefits. About all we were told about in Drivers Ed was about organ donation itself which can save lives.

    5. Are you an organ donor? Why or why not? Yes, I've been one since I first had a driver's license. I believe that when I die, I won't be needing those organs anymore and if they can help save someone's life and allow them to be able to do things they certainly can't while waiting on the list to get an organ, then I'm doing one last good thing on this earth.

    6. Did your knowledge or lack of knowledge effect your decision? If so which one and why? Nah, I'd say I knew enough about it to decide to do it, though my parents were a bit miffed at me since they are kind of paranoid about organ donation. I can say that's something to really address sometime is the worry that people have about someone pulling the plug on a person in a coma because that was the argument my parents and I had over organ donation.

    7. Do you know anyone who has donated/received an organ/tissue? Yes, a couple people. One is a good friend who had a kidney transplant after he had a serious bout with cancer. I know of others who have had transplants but that's the person who has made me know how important it is since he waited so long on a list and was on a machine and all because of it.

    8. Are your parents organ donors? If not, do you know why? No, they are not. My dad may have changed his mind and says he's considering it but my mom won't because she's paranoid that if she were in a coma, they'd unplug her and harvest her organs. She won't lose that fear.

    9. Did your parents influence your decision to be or not an organ donor? Nope, since we disagreed heavily on my decision. Of course, they also disagreed on my choice to want to be cremated instead of buried, but like the organ donation, they have no choice.

    10. Do your parents know your wishes on organ donation? Yes, and I wrote out a living will and will to ensure they follow it because my mom has a tendency to do what she feels is best and I don't want my wishes to be ignored because she's nervous or wants an 'old fashioned funeral'.

    11. Did/Are you going through Drivers Ed through your local DMV office? Or through an online course or home study? I went through Driver's Ed in high school, in 11th grade.

    12. During the course did they mention organ donation? Yes, a police officer actually came in and spoke to us a bit on it, as well as drunk driving and drug abuse.

    13. Do you have a Drivers Permit or license? License.

    14. How long when you got your permit were you given to make a decision if you wanted to be an organ donor? I had my underage license at 17, and when I turned 18, I made the choice since I could not before that due to being a minor. I would say during that time, about 5 months.

    15. Did you know before you went to the DMV that they would ask you about becoming an organ donor? I was not really asked at the DMV. There was a box to check on the form and so I did check it. No one mentioned it otherwise. There was however a poster for it.

    organ donation for new drivers? The local DMV office, Health classes, etc. Any other suggestions? I think the local DMV of course, in the Driver's Ed classes if people take those in school, Health Class is a good one too. Maybe in the hallways where they like to put up other posters for corporate stuff anyway in schools now. >.> It'd be nice to have a poster with purpose up around there.

    5. Are you an organ donor? Why or why not? Nope, never been asked by asker's initiative. And I've never taken the initiative to do so myself...

    6. Did your knowledge or lack of knowledge effect your decision? If so which one and why? Lack of it :P I only know one way of becoming one and that's getting it on your driver's lisence...

    organ donation for new drivers? The local DMV office, Health classes, etc. All around schools and on television, if the government has no problem raising awareness about cervical cancer through these two things why not also raise awareness about being an organ donor?

    I'm 22, have had my license for three years and I'm a registered organ donor. Would you like me to fill this out for you anyway or am I overqualified (probably wrong word choice...)?
