    This theory is about what happened VAT... sort of. It's pretty far fetched, so maybe you'll just enjoy a good laugh. XD
    I am just going to say that Master Xehanort needs a heart... for some reason (how's that for detailed?), and he's after Terra's. Since Terra's heart is too strong, MX has to weaken it. Therefore, he does something horrible to Ven (I'm not sure quite what, but it causes him to disappear somehow, and then causes his clone to pop out of Sora O_o). Possibly since that doesn't work, MX then goes after Aqua.
    But getting to my main point, MX weakens Terra's heart so much that it breaks. I am thinking that the pieces of his heart form the various Terra-look-a-likes in the previous KH games: Xehanort (ATW's apprentice who later becomes heartless Ansem from KH 1), Xemnas, and the Enigmatic Soldier. It seems like the Enigmatic Soldier has most of Terra's hatred, and Xemnas has most of his sorrow (Aqua's armor in the room of sleep >.>). I think Xehanort (ATW apprentice) is the chunk of Terra's heart that MX got a hold of, because when he turns into a "heartless" he is wearing MX is clothes.
    And as for Aqua, I think she is the fourteenth member. (Big surprise) I don't understand the whole "Xion" name thing, but I think it's just an alias to get into the Organization. Somebody said on another site that the "Ino" could possibly be her other name, since it's apparently the name of a Greek sea goddess... Aqua=water=sea? Bleh.
    Ignore that last sentence. On some of the other sites they are saying that the Fourteenth's hair is indeed blue. I am now almost completely sure that she is Aqua or at least a form of her. But why would she be in the Organization? Maybe she's trying to bring Terra (Xemnas) and Ventus (Roxas) back. That's my best explanation.
    And that pretty much sums it all up folks. Not a very good theory, but maybe it'll spark someone else's ideas. :D
