U need to finish all worlds, including Atlantica an 100 acre woods... then fight Xemnas again in twtnw. the original secret ending from KH2 should play. choose to save ur game after the credits, and when u next go to the world map it should say "New episode added" at disney castle. so then just go to hall of cornerstone and watch scene then go through portal to fight terra.
