Stop, everyone!!! I need to voice my stupid opinion: Everyone remember Xehanort's heartless had a Heartless Emblem for a necklace? Way to the Dawn has it for the keychain. That clears where the keychain is from. The actual keyblade, I think, was stolen from Roxas. Riku defeats Roxas with his Oblivion, replaces the keychain, and gives the Oblivion keychain to Sora. There, my dumb opinion.
