It's done through receptors and receptor proteins to receive chemical signals. The vesicles approach the cell surface and fuse with the cell membrane and it's contents. The transmitter is then released into the "intercellular space" where it can then be received by other cells' membranes. AKA communication. Correct me if I'm thinking of the wrong membrane communication.
