Maybe we're looking at this wrong. I don't know if this has been stated/theorized or not, but maybe 'Xehanort' isn't a name; maybe it's a title. Go along with me for a second: For anyone who's seen The Princess Bride, there was a character called 'Dread Pirate Roberts' (or something like that, it's been awhile). The character we see as DPR turns out to be the Princess's love interest, who returned after being kidnapped for a few years. The current DPR at the time trained the kidnappee to be the next DPR, while he lived on in retirement, as 'whoever'. My point is, maybe it's not as complicated as mind transfer; maybe somewhere down the line, Terra (or whoever) gets trained to be the next 'Xehanort', gets some kind of amnesia, and as a result, only remembers he's 'Xehanort'.
