My suggestion: Make it Impact, size...30
Color: White
Make a drop shadow, keep settings
Add a clipping mask and drag it so that the top left fills the text.
Then on the text layer, add outer glow. Make it black, set to normal, and opacity to 85%-100%
Let me knows how that works out.
(Make sure the AA is set to something other than None)

The background in interesting, that I like. What I have a problem with is the sprite. It doesnt flow with the rest of the piece. Its just kind of floating. Most sprite tags have c4d somehow connected to the sprite render. As just a background, I like it. Those two rectangles in the bottom right are a nice touch as well.

Also, the color of marly went from pink to greygreen

Yes, a person makes the decision to get cosmetic surgery to make themselves feel better. However, it is society that makes that person define perfection. The social norms and expectation is the reason why cosmetic surgery is so sucessfull. Even though there are clients who are not there to make themselves fit the societies definition of attractive, as you said burn victims, there are not enough to make it the widespread successful profession that it is today.
