Ah, I see...(no worries about the pic, I do see the cluster, more or less) but if it's come to the point of causing such pain, I'd assume that it's probably time to see your doctor. Maybe it was something simple as an irritation, but if it's lingered for month+ then it's out of your hands. Maybe Neosporin (if it's available to you that is) can help, as I've seen it work on many forms of wounds and irritations.

Well, I'm no doctor, but it may be a very mild form of eczema. But I could be wrong. Or it may be just a small (simple) irritation on your skin. It's hard to tell from the picture. Regardless, if you have been scratching it, STOP. Even if it itches severely. The number one reason an irritation takes so long to heal is because it's hard to resist scratching the itch. You can try using moisturizing [hydrating] cream (non-scented or non-perfumed and etc). Basically, don't use anything fancy. Chemicals in them can worsen the irritation if it reacts in the wrong way. You can try this method and see if the spots lessen over few days, if not...I'd recommend you see your regular doctor and see what he/she suggests.
