As far as I know, there is a colon in the title. Also, you might want to put an ampersand or a plus sign in between "Chain of Memories" and "Reverse/Rebirth" since they're technically two separate storylines.

Anyway, what's an ampersand? Also, if they're two separate things, maybe I should just title them "Kingdom Hearts: Re:Reverse / Rebirth"?

An ampersand is the "and" symbol (&). I'd take out the first colon if you're going to call it that.
