So the first three lines are all forms unlocked and the next three are the weapon mod. I got a single keyblade when I tried. Is there a special condition to work?

You can make a nearly invincible gummi ship without using codes. When making a gummi ship you only use the outer two layer of boxes and you will be invincible to all flying attacks, you will still be vulnerable to body slam attacks though. Is there any more codes like the one which changes the characters width? Just for fun. Anything would be nice.
